Explain:
Psoriasis is a chronic, relapsing inflammatory skin disease caused by a disorder in the differentiation
and proliferation of keratinocytes, accompanied by immune inflammation.
→ It is not contagious and is not an infection.
There are three main factors:
(1) Genetic – Immunological
Individuals carrying the HLA-Cw6 gene are more susceptible.
An overactive immune system: T cells (Th1, Th17) release cytokines TNF-α, IL-17, IL-23 → stimulating skin proliferation.
(2) Keratinocyte proliferation
Normal skin cell turnover cycle: 28 days
In psoriasis: 3–4 days → skin thickens, turns red, and is covered with silver scales.
(3) Triggering factors
Stress
Infections (especially streptococcal pharyngitis)
Skin trauma (Koebner phenomenon)
Medications: systemic corticosteroids, beta-blockers…
Alcohol, smoking.
Symptoms:
Typical lesions
Erythematous plaques – well-defined – covered with silvery-white scales that peel off in patches.
Common locations: elbows, knees, scalp, sacral region.
Classic signs
Psacea: scales fall off like candle wax when scraped.
Cellulose: a thin membrane is visible upon deep scraping.
Auspitz’s sign: pinpoint bleeding when scales are peeled off.
Combined symptoms
Mild to moderate itching.
Nail damage may occur: pitting, thickening, or separation of the nails.
Some patients develop psoriatic arthritis.
